My Action

Croí MyAction was developed in response to the urgent need to address the adverse lifestyle trends associated with chronic conditions such as heart disease, stroke, diabetes and obesity. In Ireland, these conditions are expected to increase by 40% by 2020, thus presenting a serious challenge for our health-care system, society and economy as a whole.

 

Croí MyAction is a unique, evidence-based programme which helps individuals and their families reduce their risk of developing heart disease, stroke and diabetes.

This FREE programme is targeted at those over 50 years of age and who have known risk factors such as family history, smoking, raised cholesterol, blood pressure and Type II diabetes. It is also suitable for those who have had a stroke, Transient Ischemic Attack (TIA) and have peripheral vascular disease. It is the first prevention initiative of its kind to be established in Ireland.

 

How does the programme work?

 

Individuals attend a 12-16 week intensive lifestyle programme which includes healthy lifestyle change (smoking cessation, healthy food choices and increasing physical activity levels) as well as management of cholesterol and blood pressure. People who join the programme are initially assessed by a multidisciplinary team which includes nurse specialists, a dietician and a physiotherapist. Following this assessment, the team set goals and objectives that are realistic to the individual and these are reviewed on a weekly basis. The programme is very much focused on achieving long-term results and individuals are offered the opportunity to have a further assessment at the end of the programme and 1 year later. The programme consists of attending an exercise and health promotion session once a week for two and a half hours. It is flexible offering a choice of both daytime and evening sessions.

What makes this programme different?

This programme is unique in that it is actively empowering people and communities through its individualised approach to behavioural change and lifestyle modification. It is family centred, actively involving the patient’s partner and other family members. It is located in the heart of the community making it more accessible to those who most need it.

Five years in – what has been achieved?

Since its establishment in 2009, the programme has reached over 1200 individuals. Year on year the programme has achieved outstanding and measurable improvements in cardiovascular health which have been widely published.  “We have seen remarkable results,” says Dr. Jim Crowley, Consultant Cardiologist. “For example, 50% of smokers have quit, the number of people achieving physical activity targets has jumped from 9% to 61%, blood pressure control has improved by 40%, cholesterol by 60% and also the weight loss results are very impressive. Many of these results are due to the significant changes people have made to their lifestyles, which in the long term will greatly reduce their risk of heart disease and stroke.”
