MyStroke Programme
The Croí MyStroke Programme is a 4-week, person-centred, educational intervention, designed to help stroke survivors and/or their family members and carers interested in gaining a better understanding of their personal stroke journey.
Programme Information
Goal of the programme:
- The goal of the Croí MyStroke Programme is to bridge the gap between discharge from the hospital and community re-engagement
- We aim to empower stroke survivors, family and carers by providing them with the resources and skills they need to manage the recovery journey, whilst fostering self-efficacy in activities of daily living and promoting long-term health and well-being
- Recognising that every stroke survivor’s journey is unique, our programme is individualised to meet each participant’s specific needs, ensuring a tailored approach to care
What to expect from the programme:
Education
- We will talk about stroke and what it does to the body and mind
- We’ll clear up some common myths and chat about the ups and downs of recovery
- You’ll learn about how stroke can affect your thinking, emotions, movement, social life and work
Practical Application
- Participants will engage in practical activities where they will work through their personalised ‘MyStroke Worksheet’
- This exercise aims to deepen their understanding of their unique stroke experience, assess the impact on their functional independence, and explore strategies to enhance their participation in daily life
- Participants are supported to develop strategies for managing their condition, including preventing future strokes and maximising their functional independence
What does the programme involve?:
- A commitment to a 2 hour session, 1 day a week, for 4 weeks (free to opt out at any time)
- A pre-programme questionnaire
- Each session will be divided into two parts with a 30 minute break for tea, coffee and biscuits
